• Box with: o One vial containing 120 mg of artesunate powder o One ampoule of solvent: 2 ml of sodium bicarbonate injection solution (50 mg/ml) o One ampoule of diluent: 10 ml of sodium chloride injection solution (9 mg/ml)
Arinate®, administered via intravenous (IV) or intramuscular (IM) injection, is indicated for adults and children in the treatment of severe malaria caused by Plasmodium falciparum.

POSOLOGY

Adults and Children Weighing More Than 20 kg • 2.4 mg of artesunate/kg body weight, administered via IV or IM injection at 0, 12, and 24 hours • Then once daily until an oral treatment can be administered • Arinate should be given for a minimum of 24 hours (3 doses), regardless of whether the patient can tolerate oral therapy before that time • After at least 24 hours of Arinate administration, the patient should transition to a full oral antimalarial combination therapy, such as Co-Artesiane or Co-Arinate Children Weighing Less Than 20 kg • 3 mg of artesunate/kg body weight, administered via IV or IM injection at 0, 12, and 24 hours • Then once daily until oral treatment is possible • Arinate should be given for a minimum of 24 hours (3 doses), regardless of whether the patient can tolerate oral therapy before that time • After at least 24 hours of Arinate administration, the patient should transition to a full oral antimalarial combination therapy, such as Co-Artesiane • Due to the instability of artesunate in aqueous solutions, the reconstituted solution must be used within one hour

ADMINISTRATION METHOD

A : Dosage calculation

The required dose is calculated as follows: • For adults and children weighing more than 20 kg: o Dose (mg) = body weight (kg) × 2.4 • For children weighing less than 20 kg: o Dose (mg) = body weight (kg) × 3 The number of Arinate vials to be used per dose must be determined before reconstituting the artesunate powder. The use of the 240 mg vial is particularly suitable for adult patients

B : Preparation of injectable Arinate (artesunate) Solution

• Draw the provided sodium bicarbonate solvent into a syringe and inject it into the artesunate powder vial • Shake the vial until the powder dissolves completely, ensuring a clear solution

C : Dilution of Reconstituted Solution

The concentration should be adjusted based on the mode of injection: • IV administration: 10 mg/ml (entire ampoule of diluent) • IM administration: 20 mg/ml (2- 4 or 8 ml of the provided diluent) • Shake well and ensure the solution remains clear

D : Injection Volume Per Dose

The volume depends on the patient’s body weight as well as the mode of administration.
